Form details:
- Jumper
Registration Form and Agreement. This
is the first step in the process. Each participant
must sign a Jumper Registration, Agreement and Release
or that person will not be able to jump. You can return
this form to your local host with your pledges or you
can mail the form and payment directly to SOAR® at
the address indicated on the form. The $600* minimum
of raised pledges is due with registration. It is
crucial that all jumpers read and understand the full
disclosure and release before signing the form.
*Minimum for licensed skydivers is $100, $300 for AFF.
If Raising Pledges
-
Pledge Payment Form. Each jumper is required
to raise a minimum of $600* in pledges. These pledges
are tax deductible to the extent allowable by law. The
success of this event as a fundraiser depends on the
number and amount of pledges collected. Each person who
makes a pledge should fill out this form. Donors can
write their credit card numbers on the form, or attach
a check payable to OPERATION FREEFALL. We strongly
encourage you to collect the pledge at the time it is
made (which is MUCH easier than tracking down donors
and collecting later). Anyone can collect pledges- they
don’t have to be skydiving to join in. SO, if you
have some friends who say they would like to help but
would rather not jump out of a plane, you should give
them some pledge payment forms and ask them to help you
raise pledges. Donors may give the pledge payment form
and their payment to you, or mail it directly to SOAR.
*Minimum for licensed skydivers is $100, $300 for AFF.
